This what's wrong image was brought into one of our dealer training sessions. Can you tell what was added on to this system?  What makes this add on an issue?

3 ton hoist overload

    Looks to be a 3 ton hoist on a 4000# max system also.

    Its hard to tell from the image, but it appears that a wide flange beam was added on to the bridge in order to accommodate a coffing 4 wheel trolley?
