In the last what's wrong post it is pretty apparent that something hit the crane system. For those that guessed forklift, you win! 

This is another what's wrong discovered in the field by a Rep that was traveling. Can you tell us what is wrong in this application photo?

    That spliced header will fail!

    End user modified the structure (poorly) to make it wider for clearance to adjacent structure. You would not find me standing anywhere near this.
